A 45-year-old female patient was admitted to the emergency department with the complaint of syncope. It was learned from the patient's history that he had a diagnosis of familial partial lipodystrophy 2 (FPLD2). A-V complete block was observed in the electrocardiography of the patient, who also had a history of insulin-dependent diabetes mellitus and coronary artery bypass. Successful revascularization was performed because severe stenosis was observed in the aortic right coronary artery saphenous vein graft in coronary angiography. Afterwards, permanent pacemaker implantation was performed as a result of the electrophysiological study performed in the patient whose syncope attacks continued. In this case, we wanted to emphasize that serious cardiac conduction defects are not only related to coronary artery disease in individuals with FPLD2 disease, but that the disease may also present with direct conduction defects.
