Coronary artery bypass graft (CABG) surgery is one of the main treatment procedures for revascularization. The diagnosis of complication may be difficult after CABG. Iatrogenic connection of the graft to the venous system is a rare complication. It causes distal ischaemia due to arteriovenous shunt. This may lead to heart failure and fatal arrhythmias. Anti-arrhythmic drug-resistant ventricular arrhythmias occurring immediately after coronary artery bypass surgery may sometimes be procedure-related. We present invasive management of iatrogenic left internal mammary artery (LIMA) to great cardiac vein (GCV) anastomosis leading to life-threatening ventricular arrhythmias.
