A 52-year-old man was admitted to the hospital emergency room with cardiogenic shock. The electrocardiogram showed ST elevation and T inversion in leads I and AVL, but no Q waves. Left ventricular aneurysm rupture at the posterolateral wall and hemopericardium was diagnosed with two-dimensional and color Doppler echocardiography. The patient was directly sent to the operating room and the defect was closed. He made a good recovery. Four months later coronary angiography and left ventriculography were performed. The coronary vessels were patent and there was 50 % stenosis at the proximal 1/3 portion of left anterior descending artery. Left ventriculography showed anterolateral and posterolateral hypokinesis. The patient was symptomfree and leading an active life 4 months later. This patient is the first published case of left ventircular aneurysm rupture salvaged surgically in Turkey.
