Extracranial carotid artery aneurysms are rare, making up only 1-4% of all peripheral artery aneurysms. A 59-year-old female patient presented with a right submandibular mass. She also had a complaint of tinnitus. There was a pulsatile submandibular mass on physical examination. Doppler ultrasonography showed an aneurysm of the carotid artery and carotid angiography demonstrated a 2-cm aneurysmal sac originating from the internal carotid artery. Surgical resection of the aneurysm was performed followed by an end-to-end anastomosis of the internal carotid artery. The patient had no complaint in the postoperative fourth year
