Coexistence of Brugada and Wolff Parkinson White Syndrome: A Case Report and Review of the Literature

A 31-year old male patient presented with complaints of palpitations, dizziness and recurrent episodes of syncope. 12-lead electrocardiogram (ECG) revealed manifest ventricular preexcitation which suggested Wolff Parkinson White (WPW) syndrome. Moreover, incomplete right bundle branch block (RBBB) and 3-mm ST segment elevation ending with inverted T-waves in V2 are consistent with coved-type (Type 1) Brugada pattern. Electrophysiological study (EPS) was performed and during mapping, the earliest ventricular activation with the shortest AV interval was found to be on mitral annulus posterolateral site. After successful radiofrequency (RF) catheter ablation of the accessory pathway, the Brugada pattern on the ECG changed which pushed us to perform ajmaline provocation test. With the administration of ajmaline, type 1 Brugada ECG pattern occured. Considering the probable symptom combinations of these two coexisted syndromes and the presence of recurrent syncope episodes of the patient, programmed ventricular stimulation (PVS) was performed and after ventricular fibrillation (VF) induced. Therefore, an implantable cardioverter-defibrillator (ICD) was implanted soonafter.
