In-stent restenosis and occlusion are common and well-known complications of the superficial femoral artery (SFA) interventions. In this conditions, treatment options are re-intervention or surgery. Percutaneous antegrade approach is an effective and a safe method for the SFA stent restenosis or occlusion treatment. When the antegrade intervention fails, the retrograde transpopliteal approach may be performed alternatively. However, there is no distal vessel flow in some patients who admit with acute leg ischaemia clinic. This condition can result in extremity amputation in these patients. In this case, we reported direct stent puncture technique to recanalize of the SFA in-stent occlusion after an initially unsuccessful antegrade intervention.
