Shone anomaly/syndrome is a rare congenital cardiac malformation characterized by serial obstructive lesions of the left-heart at multiple levels. Herein, we present an unusual case of an adult male patient with palpitation and worsening dyspnea. Echocardiographic evaluation was in favor of Shone complex associated with left ventricular non-compaction cardiomyopathy. To our knowledge, the association of non-compaction cardiomyopathy with this complex has not been previously described.
